The Practice is a small well equipped surgery that was built in 1988, located near ASDA in Silverhill.
As we are a small surgery covering the area of St Leonards surrounding the surgery, we only have a small catchment area.

How to Register
To register at the practice:
- fill out an online registration form
- visit reception when we are open and collect a paper registration form
- download a New Patient registration Form Jan ’24 and PRF1 form and bring them to the surgery.
Both these forms need to be completed & brought into the surgery with photo i.d & your proof of address.
(if you are registering children only the PRF1 form needs to be completed)
